Insurance
We accept cash, personal checks, and major credit cards, including HSA and flex spending cards, for payment. Full payment is required on the day of service. We accept most plans from the following insurance companies, but please check with your insurance to determine if we are in-network for your specific plan:
- Preferred One
- Blue Cross Blue Shield of Minnesota
- United Health
- Medica
*Please note, we are currently only accepting Blue Cross Blue Shield, Preferred One, United Health, and Medica insurance. Please also note, we do not accept Medicaid or Medicare plans.
If you are insured with a different insurance network, you may be able to request out-of-network reimbursement or preauthorization for care. In this case, we are glad to provide you with receipts which you may submit to your insurance company for potential reimbursement.
We encouraged all patients who plan to use insurance to contact your insurance provider prior to initiating services, and confirm your deductible, copay, and/or coinsurance responsibilities, as patients are ultimately responsible for services rendered.
Private Pay: No Surprises Act & Good Faith Estimate
As of January 2022, The No Surprises/Good Faith Estimate law (Section 2799B-6 of the Public Health Service Act), healthcare providers need to give clients who do not have insurance or who are not using insurance an estimate of the bill for planned services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
